WHAT IS A SUPPORTED RESIDENTIAL FACILITY?

A privately owned facility providing accommodation and support to people with disabilities, mental health disorders and the frail and aged.

 Supported Residential Facilities (commonly known as SRF’s), vary in size, and range from having 20 – 70 residents.

 We are staffed 24 hours a day, 7 days a week, and provide a home-like environment and a range of services detailed below:

 

WHAT SERVICES DO WE PROVIDE?

- Resident-focussed Care

- Accommodation – Both Single or Shared Rooms are available, depending on the facility’s     capacity.

- Quality, fresh cooked meals with a focus on resident’s dietary requirements and preferences.

- Personal laundry, linen change & room cleaning on a weekly basis

- Medication administration and supervision via Webster Packs

- Prompted and supervised dressing and grooming

- Assistance with personal finance arrangements when required

- Support to access external services on site for

·        Totally assisted Personal Care

 

  •  Wound care
  •  Continence aids
  •  Podiatry
  •  Dietitians
  •  Visiting GP
  •  Post hospital care
  •  Health care monitoring and maintenance
  •  Lifestyle activities
  •  Visiting hairdresser
  •  Psychosocial support and activities
  • Transport assistance
  • Some SRF’s also support exercise groups and audiologist visits

Please note:

Those requiring assistance with mobility by means of lift assisting devices are not suitable for         placement in an SRF support model.  

Medical/psychological and clinical services are externally sourced by the individual for their needs.

Clients who are requiring a higher level of support, ie Aged Care, will be assisted to transition into     a more appropriate service for their needs.

 

WHO LIVES IN A SUPPORTED RESIDENTIAL FACILITY?

People with a range of disabilities, mental health disorders, complex health issues, and the frail and aged. Many of our clients are socially disadvantaged vulnerable people, over the age of 18, requiring a low level of support with activities of daily living.

 A caring, and driven team of people committed to providing clients a better life.

 All staff have a Police Check / DCSI Screening, Senior First Aid Certificate, Manual Handling Training, Medication Management Certificate and are trained in Safe Food Handling.

 Additionally, staff either have Certificate 3 or 4 qualifications in Disability, Mental Health or Aged & Community Care, or are working towards achieving these qualifications.
